Day Supports programs focus on skill development in self-help, domestics, socialization, and recreation/leisure activities in order to maximize the personal growth and independence of each individual so that they may live as independently as possible in the community.
Individualized programs promote maintaining/increasing independent living skills and community integration. These include self-care, access to community services, appropriate social interactions, recreational/ leisure activities, and the ability to recognize reality-based concepts. This Day Supports program complements the Congregate Care program.
Individualized programs provide training in prevocational skills, community integration, speech/language development, and social interactions for individuals with developmental disabilities. This helps people utilize vocational opportunities and increase communication and social interactions. This Day Supports program complements the ICF/MR and Transitional Living programs.
Day Supports provide meaningful employment options for which people receive wages based on piece rates/time studies. Day Supports is provided to people in either ICF/MR, Transitional Living, or Congregate Care programs.
EXTENDED SERVICES (ES)
This program provides supports for individuals employed in the community. The support is provided on an individualized basis by a job coach. Initial placement in a job is provided through Supported Employment. Extended Services provides long term follow up.
